Prevention is the key to maintaining healthy peristomal (around the stoma) skin. If the skin becomes damaged, it could be painful and could lead to infection. It also becomes more difficult for the skin barrier/wafer to adhere to your skin.

When applying the skin barrier/wafer of your pouching system, it’s important to make sure your peristomal skin is healthy and dry. This will help to ensure the pouch stays in place in between changes.

Choosing A Well-Fitting Skin Barrier

Your stoma size will change up to 10 weeks following surgery so you’ll need to measure it periodically. Once you’ve selected the skin barrier/wafer type to use (cut-to fit, pre-cut or moldable), you’ll use a measuring guide to determine the size of your stoma. Proper fit of the skin barrier/wafer around the stoma is important in preventing peristomal skin problems.

Tips For Pouching System Changes

  • Make an easy-to-follow schedule for your pouching system. This will ensure your skin barrier/wafer is changed before the adhesive has eroded, reducing the chance of output coming into contact with your skin and causing irritation. Always empty your pouch before it is half-full. Consider using a pouch with a vent or filter for gas release.
  • How often you change your pouch depends on the stoma type, the stoma location and the type of stoma drainage. Activity, perspiration and body shape affect wear time. Our Ostomy-Care Specialists can help you find the ostomy products that are best for you and your lifestyle.
  • At each skin barrier/wafer and pouch change, make it a habit to look at the skin around your stoma. Redness, swelling or a rash are signs of irritation. If you see any such signs, notify your healthcare provider.
  • Never rip or tear off your skin barrier/wafer. Instead, remove the skin barrier/wafer gently by beginning with one corner of the barrier and slowly pulling off the remaining adhesive. Adhesive remover wipes can make pouch changes easier and ensure clean skin, ready for applying your next skin barrier/wafer. Soap and warm water can also be used. If you prefer to use soap, use one without perfumes or oils as this can cause skin irritation and also prevent the barrier from properly adhering to the skin.
  • Make sure your peristomal skin is completely dry before replacing your pouch and skin barrier/wafer. Dry skin ensures a good adhesive seal and helps reduce the risk of fungal infection.
  • In hot, humid locations, consider using a pouch with a fabric backing. This will keep the pouch from sticking to your skin and causing skin irritation.

Application Guide For A One-Piece Ostomy Pouching System

  • Cut-to-fit skin barrier/wafer:

    Prepare the area for the skin barrier/wafer by first cleaning and drying the peristomal skin. Then measure the stoma using the measuring guide.

  • Cut-to-fit skin barrier/wafer:

    Trace the measurement from the measuring guide onto the back of the skin barrier/wafer.

  • Cut-to-fit skin barrier/wafer:

    Cut the opening in the skin barrier/wafer.

  • Cut-to-fit, pre-cut and moldable skin barrier/wafer:

    Remove the protective backing from the adhesive by gently separating it.

  • Apply pouch by centering skin barrier/wafer opening around the stoma.

    For Moldable Skin Barriers/Wafers: Gently press and mold the opening so it fits snugly against the stoma.

  • Secure to skin by applying gentle pressure for 30 seconds, paying particular attention to the area closest to the stoma. If the pouch is drainable, ensure the base of the pouch is properly closed/secured.

Application Guide For A Two-Piece Ostomy Pouching System

  • Cut-to-fit skin barrier/wafer:

    Prepare the area for the skin barrier/wafer by first cleaning and drying the peristomal skin. Then measure the stoma using the measuring guide.

  • Cut-to-fit skin barrier/wafer:

    the measuring guide onto the back of the skin barrier/wafer.

  • Cut-to-fit skin barrier/wafer:

    Cut the opening in the skin barrier/wafer.

  • Cut-to-fit, pre-cut and moldable skin barier/wafer:

    Remove the protective backing from the adhesive by gently separating it..

  • Center the skin barrier/wafer around the stoma. Secure to skin by applying gentle pressure for 30 seconds, paying particular attention to the area closest to the stoma.

    Cut the opening in the skin barrier/wafer.

  • Place pouch onto the skin barrier/wafer starting at the bottom of the click lock security ring, as shown. Gently press into place while moving toward the top of the skin barrier/wafer with both hands. You should hear an audible ‘click’ when both pieces are in place.

    If the pouch is drainable, ensure the base of the pouch is properly closed/secured.
